Self Report Questionnaires Online

NEW ! The Self Report (SR) Questionnaire Booklets are now available online. While we can still distribute and accept pencil-and-paper booklets for those who prefer them, electronic questionnaires will be available on the Internet for your convenience.

To begin your SR Questionnaire, click on the appropriate button below. Your Participant ID number will serve as your login name and your birthdate as your password. Be sure to read our Frequently Asked Questions (FAQ) before beginning, and contact us if you have any questions.

Answers to FAQ

1. Who can use the online SR Questionnaires?

The online SR Questionnaires are designed for participants of the Police Stress and Health Program research studies who have access to the Internet via a web browser (such as Netscape, Internet Explorer, Mozilla, Safari, etc.).

At this point, only police trainees and officers at certain time points will have access to SR Questionnaires online. We are currently working on creating online SR Questionnaires for officers enrolled at other time points, as well as for our enrolled Spouse/Partners.

Please bear with us as we work on getting these sites up. In the meantime, please keep checking this page for updates.

2. How do I log in to my online SR Questionnaires? What is my username and password?

You may access the online SR Questionnaires from any computer with a web browser and Internet connection by clicking on the appropriate button above. You may want to consider bookmarking this page so that you can find it easily in the future.

Your username is the unique ID we assign to you when you first enroll in the study. The ID numbers are six digits and usually begin with the digits 31, and are listed on the Booklet Preferences Form you receive during your initial appoitnment. If you are unsure of your unique ID, please contact us and we can provide that information for you.

Your birthdate serves as the password into the online SR Questionnaires.

3. Is my personal information safe?

Maintaining the confidentiality of your responses is very important to us. We have taken a number of steps to insure your privacy.

As with all components of the Police Stress and Health Program studies, no identifying information is ever associated with the responses you provide to questions in booklets or during interviews. A unique ID number is assigned to each participant, and that ID number is the sole means of identification used on individual questionnaires.

Furthermore, information collected online is stored in a secure environment, behind a firewall, and is accessible only by study personnel. Even then, the information you provide is marked only by ID number.

4. I use a shared computer. How do I keep my information private?

When using a shared computer, always be sure to log off from the online SR Questionnaires web site and close down your web browser entirely. This will ensure that other users do not hit the 'Back' button to access your information.

5. I don't have the time to complete all of the questions at once. What do I do?

We understand that there are a lot of questions to answer. We have found that it typically takes a participant between 1-1.5 hours to complete all of the questions presented.

If you do not have the time to complete all of the questions in one sitting, you may close the web browser at any point. When you log back in, the questions will pick back up where you left off.
